To apply for a Foundation grant, a nonprofit organization must submit the completed program application to the Foundation by the corresponding deadline. All required forms and worksheets can be downloaded from the menu on the right side of this page; the documents are available as Word and Excel, or PDF files.
Before completing your grant application, it is highly recommended that you speak with a Foundation program officer about your project.
The following completed items must be submitted to apply for a grant:

A Grant Application - must use Foundation form

- Line-Item Program Budget Worksheet - must use Foundation worksheet

Outcomes Worksheet - must use Foundation worksheet

IRS determination letter stating current 501(c)(3) tax status (not State of Florida sales tax exemption)

Audit or review (most recent, not more than two years old) attested by a Florida-licensed CPA (required for all grant applications over $10,000)

Operating statement with budget comparison (most recently completed fiscal year)

List of organization’s current Board of Directors, their addresses, and occupations

Price quotes from three separate sources for items exceeding $1,000 (list in Line-Item Budget Worksheet)
Additionally, the following items are suggested as part of any application:

If this project is a collaboration or cooperative effort, attach all letters of support, agreements, contracts, memos of understanding, etc.

Fundraising Plan (for this project) - must use Foundation worksheet

Critical Event Timeline (for this project) - must use Foundation worksheet
Submission instructions One completed original application, paper-clipped (not stapled or bound), along with all required items and any suggested items (in numerical order above) must be hand-delivered to the Foundation by 4 p.m. by the deadline date. Incomplete applications, faxes, e-mails, or other electronic formats will not be accepted.
